This application in the link below is divided into three parts that must be completed before submission: 

Part One (Student/Applicant) needs to be filled out and signed by the parents

Part Two (Pledge of Employer) will be filled out by the employer

Part Three (Physician Physical) on page two requires a physical with the physician’s signature (unless a current physical form is on file in Athletics). This would need to be within one year of application.

When all three parts are completed, return the forms to the Tippecanoe High School office with either the student's (a) driver’s license, (b) temporary permit, (c) state ID, or (d)  birth certificate. 

The application will be processed at the earliest convenience of the high school secretary.
